Registered Nurse - Home Health (Saturday & Sunday Only)
We are currently seeking Registered Nurses for home health visits; to work Saturday's & Sunday's to do opens in the Ventura, Oxnard, Camarillo, Port Hueneme, Santa Paula, Fillmore, Ojai.
Must be available for all these cities.
JOB SUMMARY DUTIES AND RESPONSIBILITIES Complete initial and on-going comprehensive assessments of the patient's needs including OASIS assessments at appropriate time points.
Revise the plan of care as necessary.
Initiate appropriate preventative and rehabilitative nursing procedures.
Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs.
Refer to other services as needed.
Coordinate all patient services.
Inform the physician and other appropriate staff of changes in the patient needs / condition.
Support the patient's and family's cultural beliefs.
Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
Demonstrate the ability to correctly use and direct the use of medical supplies and equipment.
Plan for discharge from service.
Participate in orientation, in-service education, utilization review and patient care conferences.
Prepare and submit, as directed, all case documentation, including clinical and progress notes, and required payroll information according to Agency policy.
Consistently display a professional appearance.
Interact with internal and external customers using positive and professional approach.
Assume responsibility and display reliability for the completion of all tasks, duties, communications and actions.
Support the mission, vision and goals of the Agency.
Maintain professional growth and development.
Participate in performance improvement activities.
Accept other duties as assigned.
PHYSICAL DEMANDS AND WORKING ENVIRONMENT EDUCATION, EXPERIENCE, AND TRAINING Current, active license in the State of California by the Board of Registered Nursing.
Current CPR, health clearance and clinical skills self-assessment.
Successful passage (80%) of Agency competency exam and core competency per skills checklist.
Minimum of one year of professional nursing experience.

Estimated Salary: $20 to $28 per hour based on qualifications.
