Licensed Vocation Nurse :
Private Duty Various Shifts Available:
3pm to 11 pm 12 hour shifts Throughout the years, the Assisted care team has offered high quality, compassionate home health services to patients and their loved ones.

Assisted offers our Licensed Vocational Nurse's a unique employment package:
Flexible hours and working close to home DUTIES AND
Responsibilities:
Ensure quality and safe delivery of home health nursing services.
Assist the registered nurse or physician in performing specialized duties.
Communicate with RN Clinical Supervisor on a regular basis regarding changes in the patients condition and/or needs.
Perform the nursing care plan procedures as directed by the registered nurse including nursing services, treatments and preventative, and rehabilitative procedures.
Perform procedures at a level of care standard within the medical community, scope of practice consistent with the Vocational Nursing Practice Act, and follows Agencys current policies and procedures.
Observe, record and report to the physician and/or clinical supervisor the patients reaction to treatment, including drugs, and any changes in the patients condition.
Assist the patient in learning appropriate self:
care techniques as appropriate.
Participate in orientation, in:
service education, utilization review and team conferences, and performance improvement activities.
Participate in patient care conferences.
Prepare and submit, as directed, all case documentation and required payroll information according to Agency policy.
Assist with discharge planning as appropriate.
Consistently display a professional appearance and conduct in all settings.
Interpersonal and interagency relationships are to be positive and professional at all times.
Assume responsibility and display reliability for the completion of all tasks, duties, communications and actions.
